We have an established a Contact Form  that you may use to communicate with us:

• General communication
• Questions and clarifications regarding clinical/nursing questions, new patient inquiries, supplement requests, billing inquiries, and scheduling/patient care requests
• Straightforward, non-emergency, and non-urgent questions to your provider

You can also use Schedule Appointment Form to schedule an appointment with us.

The Contact Form provides a secure, encrypted alternative to email for electronic communications with your medical provider. Your secure messages can be read only by Karisma Health staff members.
